NEW Wellness Incentive Period: 9/1/2024 to 8/31/2025
for
Deductible Discount effective 1/1/2026
IMPORTANT NOTE: 1/2 Wellness reward will no longer be an option.
To earn a reduced deductible for Employee + Spouse and Employee + Family coverage, employees and their spouses
who will have medical coverage in 2026 must EACH complete an annual physical and a health assessment.
Otherwise, NO reduction to the deductible will be earned.
