KMS Athletics Packet

- Must be completed in its ENTIRETY and submitted to Mr. Reed in Room 600 or the Front Office to the attention of Mr. Reed.

- Be sure to include your insurance carrier AND policy number at the bottom of page 1 on the EL3 form. Packets without this info will be considered incomplete. (Please note: School insurance policies are available for those who do not have outside insurance coverage.)

-Once complete, this packet will cover participation in all sports all year long.

-Physicals MUST be completed on the EL2 form only; no other documentation will be accepted.
